Results are arithmetic estimates or editable drafts based on information you enter and published 2026 federal references. They do not interpret your plan, decide coverage, determine medical necessity, or account for every exclusion, state rule, territory rule, or circumstance.

Appeal deadlines and coverage rules carry real consequences. Before acting on any figure here — especially a filing deadline — confirm it with your plan, your denial notice, or your state insurance department.

Appeal-routing scope

The routing aid lists the 50 states, Washington, D.C., and five U.S. territories, plus an out-of-scope option. A location and a plan label such as fully insured or self-funded can narrow questions to ask, but they do not determine the regulator, filing destination, eligibility, or deadline for a particular claim. People outside the listed U.S. jurisdictions must use their own plan documents and applicable regulator.

Accuracy

Considerable effort goes into keeping figures correct, and sources are documented on the method page and Editorial Policy. Even so, rules change, published figures get revised, and software contains errors. Content is provided "as is" without warranty of any kind. You are responsible for verifying anything you rely on.
